The MoMobile (Maternity Care Coalition)
Maternity Care Coalition's signature program provides case management, family support, and health education to pregnant and parenting women through the babies 1st birthday. Community health workers make home visits, linking women to prenatal care, behavioral health services, nutrition programs, education, and other community resources. Nurse-Family Partnership
For first time low-income mothers before 28 weeks gestation. Each mother has her own nurse who visits during the pregnancy and continues until the child is 2 years old.

Health Intervention Program (HIP) for Children with Special Health Care Needs
HIP helps parents and caregivers cope with their special needs child's day-to-day health problems by working with families in their own homes to help create a personalized care plan. Services are provided by a team that includes a nurse, social worker, parent-professional and counselor - Following are referral areas:

The Home Visiting Program
Provided by the Health Federation of Philadelphia- Serves women and children via an intensive home based program for pregnant and post partum women with children under 3 years old. Educating Communities for Parenting

Safe Haven

Maternal Wellness Center
