\n— Accompany families to doctor\u2019s appointments
\n— Explain treatment plans to families

\nEligibility <\/strong>
\nChildren up to age 21 must have Medicaid coverage and specific medical and\/or behavioral conditions like:
\n— Asthma
\n— Obesity
\n— Autism
\n— ADHD
\n 
\nHow to Apply<\/strong>
\n— Different sources (social worker, hospital, school, community-based organization, mental health specialist, or other healthcare provider) can refer you to Health Connections<\/strong> by emailing this form<\/a><\/strong>
\n— Attach copies of your child’s most recent physical and\/or psychological evaluation\/Individualized Education Plan (IEP) in order to support eligibility.
\n— You must consent for the referral to Health Connections
\n
