Nationwide Children’s Hospital (NCH) Asthma Express

Home-visiting program designed to assist patients and caregivers in understanding and managing asthma. Nurses come to your home and perform a physical assessment, determine environmental triggers, review medications and provide education. Staff are on call 24/7 to answer questions from enrolled patients and families. Services are available in Delaware, Fairfield, Fayette, Franklin, Hancock, Hocking, Knox, Licking, Lucas, Madison, Muskingum, Perry, Pickaway, Ross, Union, and Wood counties. The program is covered by most payor plans, including Medicaid. A referral from a healthcare provider is required. Providers can call the number below to place a referral.
