Mom’s Meals

Provides home-delivered, medically tailored meals to help people manage chronic conditions, recover from illness, or continue to live independently. Menu options include diabetes-friendly, heart-friendly, renal-friendly, halal, kosher, vegetarian, lower sodium, pureed, high protein or general wellness. Meals may be provided for free or at a discount for customers who are over 65, disabled, being discharged from a medical facility, or are on a Medicaid / Medicare Advantage plan that offers a paid meal benefit. Customers may also choose to self-pay for meals.  Mom’s Meals cannot determine eligibility for customers. You must contact your Medicaid provider of Area Agency on Aging to submit an authorization on your behalf to use benefits. Case managers may also enroll clients. Mom’s Meals also offers nutritional supplement delivery and nutritional counseling.
