CHOP Transition Support Programs

CHOP’s “REACH” programs are designed to help families, and young adults with disabilities, learn how to transition to adulthood. REACH’s mission is to prepare youth 14 to 24, with special healthcare needs, for a successful transition into adulthood by providing peer support, guidance and resources. The Special Kids Network

A free, statewide service for families with children who have special needs.
